Your first visit to the center is very important to us. We would like to get to know you and learn about your medical history along with a comprehensive examination. Generally once a diagnosis has been made, you have been given all of the information necessary to make an informed decision on surgery. A subsequent appointment will be given for your procedure. Sometimes in complicated situations involving surgery under local anesthesia your procedure may be performed on the day of consultation. We prefer to keep an orderly flow to our office routine, but strive to accommodate any patients in pain or dental emergencies.
In order to expedite your visit with us please provide us with the following information at your appointment:
| |
|
|
| |
- Your surgical referral slip and x-rays if possible
- A list of medications you are currently taking
- If you have medical or dental insurance, please bring the necessary completed forms. This will save time and assist us in processing your claim
